Patient with complex needs - Experience in implementation of LSV-Carewell platform

This paper described a study being undertaken to evaluate an at-home telecare platform, as compared with standard medical care, in 100 patients aged 65–85 years in Poland with at least two chronic diseases (ie hypertension, diabetes, COPD, heart failure). Depending on which diseases were present, patients were supplied with measuring equipment allowing them to assess health parameters at home; for those with COPD, this involved a pulse oximeter and the Vitalograph Asma-1 peak flow meter.
